ADMISSION
For admission, please bring the following items or documents:
- Hospital registration card (Kartu Pasien) or appointment card if any
- Identity card (KTP), or passport if foreigner or birth certificate
if patient is under 17 years old.
- Company's Guarantee Letter and/or Insurance Coverage proof
- Doctor's referral letter with medical prescription if any
- If prior appointment has been made, please arrive promptly. Rooms
are not guaranteed beyond the schedule agreed upon.
FINANCIAL ARRANGEMENTS A. Independent/Private During admission:
You are required to deposit a certain amount (depending on the medical
treatment).
Payments are made to the cashier in the form of cash, check and/or
credit card.
Patient is escorted to the room for observation or treatment.
During discharge:
No discharge on SATURDAY. On Public Holidays up to 11:30 AM only.
- Receive BLUE CARD from the nurse and proceed to the Patient's Business
Office (PBO) to settle billing.
- Receive GREEN CARD
- Escorted to collect medicine at Pharmacy

B. Partner Company Employees/Members
During admission:
- Present guarantee letter or Identity Card (on emergency cases, may be
submitted on the next working day)
- Patient is escorted to the room for observation or treatment.
During discharge:
No discharge on SATURDAY. On Public Holidays up to 11:30 AM only.
- Receive BLUE CARD from the nurse and proceed to the Patient's Business
Office (PBO) to settle any amount in excess, not covered by the company
- Receive GREEN CARD
- Escorted to collect medicine at Pharmacy
- Discharge
